Too Hot or Too Cold
If the temperature runs too high for long, development speeds up and chicks can hatch early or weak. If it runs too low, development slows and the hatch is delayed or fails. Short swings are usually fine, since the egg holds heat, but sustained errors are the most common cause of a poor hatch. Place your thermometer at egg level, not at the top of the incubator, where readings can differ.
